Student ID Card

As a student at CHC, you will be provided with a Student ID Card. This card will show your name, course details and attendance status (full-time or part-time), and will bear the photo that you are required to submit to Student Administration (details below). For eligible students, it will also include the Queensland Rail logo, which will entitle you to public transport travel in Queensland at the student concession rate.

CHC requires a passport compliant photograph to be submitted. Please see the guidelines for passport compliant photos at the Australian Passport Photo Guide.

The ID card is needed for borrowing from the CHC Library and for entering scheduled examinations.

ID Cards will be available for collection from Student Administration. Student Administration will notify you when this is available for collection. Please sign the strip on the reverse side of the card as soon as you receive it.

The ID card is valid for the duration of your course. If you lose your card, please complete a Request for Replacement Student ID Card form, which is available on the CHC website or from the CHC Reception. A replacement ID card carries a fee of $15, which is to be paid with the submission of the completed form to the CHC Reception.
