This form is to be used by new students to apply for admission to CHC postgraduate courses except the Master of Teaching (Primary) and Master of Teaching (Secondary), application for which is to be made through QTAC.

Current CHC students may use this form to apply to transfer to postgraduate courses offered by a different CHC School to their original course and/or apply for admission to a second postgraduate course. Current students who wish to transfer to a postgraduate course offered by the same CHC School as their original course are to use the Application to Transfer Course form. This form can also be used by current CHC students who have completed an undergraduate course within the past two years and who wish to apply for admission to CHC’s Master of Teaching (Primary) or Master of Teaching (Secondary) courses.

Do not complete this form if you are an overseas applicant. Please use the relevant Application for Admission (Overseas Students) form. Closing dates for 2019 applications are as follows: Semester 1 Graduate Certificate in Management (MBA Pathway): 14 January 2019 All other courses: 18 February 2019 Semester 2 Graduate Certificate in Management (MBA Pathway): 10 June 2019 All other courses: 15 July 2019

    By clicking the Submit button below I am declaring that the information contained herein is, to the best of my knowledge, correct and complete in every detail. If admitted to CHC, I undertake to comply with the rules and standards of conduct as expressed in CHC policies. I understand that:

    • CHC collects, stores and uses personal information only for the purposes of administering student and prospective student admissions, enrolment and education. The information collected is confidential and will not be disclosed to third parties without my consent, except to meet government, legal and/or other regulatory requirements;

    • CHC is collecting additional information in this form for the purposes of assessing my entitlement to Commonwealth assistance under the Higher Education Support Act 2003 and allocation of a Commonwealth Higher Education Student Support Number (CHESSN) to me;

    • CHC will disclose this information to the Department of Education and Training for those purposes;

    • The Department of Education and Training will store the information securely in the Higher Education Information Management System (HEIMS);

    • The Department of Education and Training may disclose the information to the Australian Tax Office; and

    • CHC and the Department of Education and Training will not otherwise disclose the information without my consent unless required or authorised by law.

    • CHC will provide contact details to the Department of Education for the purpose of national student surveys.
